Can zirconia teeth cause excessive wear?

With the development of dental materials and the improvement of people's aesthetic needs, zirconia restoration has become the mainstay of all ceramic restoration in recent years. Especially in recent years, with the development of high-permeability and ultra transparent zirconia, these zirconia materials gradually make doctors and patients choose them as single material full crowns (commonly known as: full zirconia crowns) because of their increasingly improved light transmittance, aesthetic effect and their own excellent fracture toughness and flexural strength.

All zirconium crown has many obvious advantages by virtue of its material itself, such as good strength, high hardness, good wear resistance and so on. However, many doctors believe that zirconia materials will have high wear on natural enamel and even other jaw restorations because of their higher hardness and strength.

But is this really the case?

Zirconia is indeed a very hard material. People may intuitively think that the higher the hardness, the more abrasion will be caused to the enamel. However, the result of wear is mainly determined by the smoothness of the material. A smooth surface will not cause excessive wear, because there is not much mechanical interlock between two worn objects.

But what is the long-term effect? Is the surface of zirconia still smooth after long-term use?

In order to investigate this problem, the University of Alabama at Birmingham (Figure 2) investigated the wear behavior of accelerated aging zirconia materials on enamel: "the zirconia after aging simulation has a similar roughness and wear degree as before. At the same time, the wear of zirconia before and after aging on enamel is less than that of decorative porcelain crowns or natural enamel.

So, how should zirconia be polished clinically?

The left picture shows the differential image of the zirconia crown pair of molars (the first mandibular molar as an example) from the initial state to the 12-month follow-up measurement. Software surfer 9 (Golden software, Co, USA) was used to analyze the wear amount of the ground tooth replica. The most severely worn areas are orange and red.

The main conclusions of this clinical report are:

✓ the measured abrasion of enamel and all ceramic is similar to that of other studies, and within the normal physiological range.

✓ there is no significant difference in the abrasion amount between enamel and all ceramic.

✓ zirconia full crown will not cause excessive wear to occlusion in 2 years.
